Students requesting to go home
- For medical reasons must come to the clinic for a nursing assessment BEFORE calling a parent or guardian.
- It is clinic policy that the nurse or clinic assistant dial the phone number and speak with
the parent or guardian giving the permission.
- The authorized person picking up the student must be listed on the Emergency Card or
person designated by parent to pick up (must have id).
- They must sign in at the front desk and obtained a visitor's pass before coming to the clinic.
- Students who use their cell phone will be referred to their Assistant Principal.
5 minute Early Release Passes
- for injury or surgery can be obtained from the nurse.
- All 5 Minute Passes REQUIRE a doctor's note stating the reason for the pass
and the number of days the pass is needed.
Medication
- For students taking prescribed medication at school, parents or guardians must bring
the medication to the clinic in the original container and sign a Medication Authorization
Form before the nurse can administer the medication.
- District policy states STUDENTS ARE NOT ALLOWED TO TRANSPORT PRESCRIBED MEDICATIONS.
- Students are allowed to have over the counter medication in original container with them.
The medication should only be taken in the clinic.
- END OF SCHOOL YEAR: Any medication not picked up by the parent will be destroyed the day
after school is out for the summer.
Asthma Inhalers and Emergency Medications
- Students who need to carry inhalers must have on file in the clinic "Permission to
Carry an Inhaler" form filled out by the student's doctor and by the parent.
Or a doctor's note specifying the need to carry an inhaler.
